Sexual Health Services

Integrated Sexual Health Services remain available during lockdown. They continue to provide urgent and some non-urgent care to service users, however, there are a number of temporary changes in response to the coronavirus Covid-19 outbreak.

There currently isn’t the option to ‘walk-in’ and if you wish to access an appointment you will be triaged via their Information and Booking line: 0800 328 3383, Monday to Friday, 8am – 5pm.

You will be assessed over the phone and priority patients are given a time for either a face to face or a telephone consultation appointment.

Booked appointments can be made for the following clinics only: Chesterfield; Derby; Buxton; Swadlincote and Long Eaton. Other peripheral clinics are currently closed.

Nursing Associates

Nursing Associates and Trainee Nursing Associates are new roles within our Primary Care Network who will be working across all our Erewash practices. They work with our other clinical staff e.g. GP’s, Nurse, Pharmacists, Health Care Support Workers to deliver care for our registered patients.

Nursing Associates work across all four fields of nursing, including:

Adult,

Children,

Mental Health,

And Learning Disability.

The main duties will include:

Undertaking clinical tasks including blood tests and ECGs,

Performing and recording clinical observations such as blood pressure, temperature, respirations and pulse,

Discussing and sharing information with registered nurses on a patients’ condition, behaviour, activity and responses,

Ensuring the privacy, dignity and safety of individuals is maintained at all times,

Recognizing issues relating to safeguarding vulnerable children and adults.

Travel Vaccinations

If you require any vaccinations relating to foreign travel, please contact the practice directly who will arrange a telephone appointment with the practice nurse to discuss further.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

First Contact Practitioner (FCP)

The role of First Contact Practitioners (FCPs) in Primary Care is to assess patients with soft tissue, muscle and joint pain and to decide on the most appropriate management pathway.

FCPs are physiotherapists with expertise in the assessment and management of Musculoskeletal (MSK) conditions.

They may also be known as Advanced Practice Physiotherapists (APP) or MSK Practitioners.

Inclusion criteria for the Erewash FCP service:
  • Over 18 years of age
  • Musculoskeletal
  • Soft tissue injuries, sprains, strains, sports injuries
  • Arthritis (any joint)
  • Possible problems with muscles, ligaments, tendons or bone
  • Spinal pain including lower back pain, mid back pain and neck pain
  • Spinal-related pain in arms or legs, including nerve symptoms e.g. pins and needles
  • Post-orthopaedic surgery
The FCP will:
  • Assess you and diagnose what’s happening
  • Give expert advice on how best to manage your condition
  • Refer you on to specialist services if necessary

Pharmacy Services

What can your Pharmacy do?

All pharmacists train for a minimum of 5 years, in medicine, minor illness and well-being, so therefore can offer clinical advice and over the counter medicines on a range of minor illnesses, such as coughs, colds, sore throat, bowel issues, aches and pains.

They do not require appointments, and have access to private consultation rooms, to help you discuss issues in private. If symptoms suggest it is more serious, pharmacists have the correct training to get you the help/ appointments you need, for example, GPs or nurse etc.

Help take the pressure off the reception and get the help you need faster.

The On Day Service

The On Day Service is running in the areas of Ilkeston and Long Eaton to improve access to primary care.

Patients who call their GP for an appointment may be directed on that same day to the On Day Service where they will be seen by a GP or an advanced nurse practitioner as appropriate.

The On Day Service is available from 8am to 6.30pm on weekdays, excluding bank holidays. To use it, patients just call their normal GP number.

  • Making it easier for patients to receive a same day consultation with the relevant healthcare professional
  • Increasing capacity and reducing waiting times for patients
  • Practices working together to offer faster more responsive access
  • Giving patients improved, quicker access to the healthcare they need
  • Reducing A&E attendances

Well Person Screenings

Everyone between the ages of 40 and 74 will be offered a health check every five years and over 75’s will be offered one every 3 years. At the check the risk of heart disease, stroke, kidney disease and diabetes will be assessed and you will be offered support and advice to help reduce that risk.

Smoking Cessation

All our doctors and nurses offer advice and support to patients trying to give up smoking.

Our nurses provide smoking cessation appointments where they provide advice, support, monitoring, nicotine replacement and advice on other techniques to help you quit.

Pneumonia vaccinations

Anyone who is aged 65 and over, persons in special categories eg diabetes, heart disease, asthma, COPD and who have had a splenectomy, are advised to have this vaccination. This is a one off injection which will give life long cover.

Influenza (flu) vaccination clinics

The flu vaccine is offered yearly to certain population groups, this is normally offered from September/October until February the following year.

You are eligible for a FREE flu vaccine if you:

  • Are aged 50 or over
  • Have a long term health condition such as a heart problem, diabetes etc.
  • Have learning difficulties
  • Are the main carer of an older or disabled person
  • Are a frontline health or social care worker
  • Are pregnant
  • Are a child of a certain age
